Requirements for Issuance of New Passport and Report of Birth
1. Duly accomplished Report of Birth Form (4 original copies)
2. Arabic Birth Certificate of child with KD5 stamp from the Ministry of Foreign Affairs with English translation (1 original and 3 copies each)
3. Marriage Certificate of Parents:
• For couples married in the Philippines, please attach the DFA-authenticated NSO marriage certificate
• For couples married in Kuwait or Ministry of Justice, present copy of Report of Marriage from the Embassy
• If marriage in Kuwait has not been reported to the Embassy, please proceed to the proper Consular Officer first and apply for Report of Marriage
4. Passport Copies, Kuwait Residence Visas and Kuwait Civil IDs of Parents (4 photocopies)
5. Duly accomplished E-Passport Application Form
6. Affidavit of Delayed Registration of Birth of child, if child has not been registered with the Embassy for more than one year and above
